Getting Started
Consultation #1 and #2 must begin by my 30 minute examination of your completed Health Questionnaire. You may fax or email the completed form back to me, along with any other relevant diagnostic test reports.
Initial Comprehensive Evaluation
Includes my pre-appointment review of your completed health history questionnaire. Factors include personal/familial health history, diet, lifestyle, stress management, exercise patterns, medication/nutritional supplement review, medical diagnoses/health problem symptomology, recent medical lab reports, and your personal health goals. Includes all of my recommendations, orally and in written form.
Initial Comprehensive Evaluation (above), plus Saliva Hormone Panel/Metabolic Profile results consultation.
You must purchase your chosen test first, which is a separate fee. See complete list and descriptions under Assessment Tools.
*You must purchase and complete your chosen assessment first. Once I receive those results,I will e-mail you and we can then schedule the two appointments times we'll need.
General Consultation by phone

1/2 hour $40 (minimum charge)
Follow-Up Consultation package
Two, 30 minute sessions by phone or e-mail. Must be used within 3 months of purchase...$75
* I strongly encourage you to purchase this package along with options #1 or #2. There is a lot of information to assimilate and work with.

Ask me any health-related question, and I'll give you the most thorough answer I can within the 20 minute time allotment. Be sure the scope of your question is not overly broad, so that I can answer more effectively.
